1. Click on "Registration Forms"
2. Click "Biological Materials Safeguards Application"
3. At the very top of the screen, mouse-over "Add/Amend/Renew Biological Materials Safeguards Application".
- Four options will appear.
4a. For amendments: Click the second option "Amend or Change an Existing Biological Materials Safeguards Application"
4b. For Renewals: Click the third option, "Renew an Existing Biological Materials Safeguards Application"
4c. To terminate existing protocols: Click the fourth option, "Terminate an Existing Biological Materials Safeguards Application"
5. After making a selection, find the previously approved BMSC application you would like to modify, then click "Select"
